
Building Strong Community Connections
Building strong connections between your students and local or state agencies is essential for promoting postsecondary success, especially among transition-aged students with special needs. Numerous valuable programs and supports are available at both the local and state levels, many of which your students may be eligible to access. Facilitating collaboration among all service providers can greatly enhance outcomes for students with autism and other unique needs. Community transition teams—including teachers, state agency representatives, school case managers, administrators, and transition coordinators—play a key role in supporting students as they prepare for life beyond school. Additionally, professional development for personnel, staff, parents, and the community can provide the education necessary to strengthen community transition teams and further support positive postsecondary outcomes.
